Polk County Sheriff's Traffic Crash investigators are seeking eyewitness accounts to the traffic crash involving a Polk County ambulance from ALS-12 which occurred on Monday, August 23, 2004.

Around 5:25 p.m. that day, the ambulance was headed westbound on Dundee Road, also known as CR 542. The ambulance had its lights and sirens operational and was moving in and around traffic. After clearing through the intersection at Carl Floyd Road near Winter Haven, the ambulance left the roadway, flipped, and hit a pine tree on the north side of the road. The crash proved fatal to Polk County Firefighter Ben Lang, and seriously injured the paramedic on board, Mike Tomlinson.
