Polk County Sheriff's Office Traffic Crash investigators are seeking witnesses to a fatal crash that occurred on December 9, 2004.

At 6:30 a.m. that morning, a tan Chevrolet Ventura minivan was headed south on U.S. Hwy 27 when it crashed into a green Chevrolet pick-up truck which was headed north on US 27 and attempting to turn left, or west, onto Deenstill Road. The crash occurred in the intersection of US 27 and Deenstill (also known as CR 54). The truck was hit on the passenger side and spun, ejecting its driver onto the road. The driver was killed on impact. Another vehicle, a tan Chrysler Sebring traveling southbound on US 27, then hit the truck in the intersection just after the original crash occurred.
